Understanding Opioid Use Disorder

To understand the importance of medications for opioid addiction, it's good to know how opioids affect the brain. Opioids such as oxycodone, heroin, fentanyl, Percocet, and Vicodin attach to specific receptors in the brain, igniting neurotransmitters and sending signals that block pain, slow breathing, and promote a feeling of calm.

 

When misused, opioids flood the brain with dopamine—the "feel-good" chemical that sends the brain feedback about rewards—creating euphoric feelings. Our brains are naturally wired to repeat behaviors associated with pleasure or reward, and when that reward system is over-stimulated by the effects of opioids, the brain remembers that behavior and records it as something that should be repeated.

 

Once misuse leads to addiction, a person continues to use the substance despite negative consequences in health and personal lives. The brain's reward center decreases in receptiveness and less and less dopamine is released with each continued substance use. At its end state, a person no longer uses the substance to feel good; they use the substance to feel normal.

Applied to our model, the goal of the Provider is to provide client evidence based evaluation and management, drive the overarching solution for the client, and manage evolving treatment plan requirements including ASAM level of care. The Pharmacist complements the Provider by providing a well of knowledge on the pharmacokinetics (movement of drugs through the body) and pharmacodynamics (body's biological response to medications and drugs). This gives the Provider an ongoing feedback loop that improves the level of information the Provider has a hand to administer the highest level of care to our clients

·         Serves as another component of encouragement for compliance following a similar reward schedule for graduating phases of treatment.
